The Basics
DT-STSD, or Virgo, grants 3 T-spin doubles. It looks mainly similar to a DT Cannon:

It is a highly greedy build that I do not recommend. It is also hazardous. It grants 3 T-spin doubles.
Here is how to carry it out:

Make the first T-spin double.

Then the second T-spin double.
And finally, the third T-spin double.
Common Mid-Game Chances
Here is how to make it mid-game:
1. JJ Base
The JJ combination is the easiest configuration to make the DT-STSD’s base (left side below):

Spot something like this mid- game during 9-0 stacking.
Stack 2 Js as the base and create the rest of the DT- STSD.
2. SZ Base
Combining S and Z to make the base (the left side below)is another common combination:

This is subtler compared to the JJ base method. Spot this.
You must stack S and Z in this configuration as the DT-STSD’s base.
3. Sky Prop Upgrade
In a more subtle manner, you can place S and Z below to make a Sky Prop composite prop. Then, top it with a J to make the DT-STSD’s structure:

Wait for this 9-0 stacking chance.
Stack S, Z, and J together as the base. This is an upgrade from Sky Prop, made using S and Z pieces.
4. 9-0 Stacking Situations
From my experience, the easiest way to create the DT-STSD is this situation during 9-0 stacking:

Place Z and J to make the base. Stack the lavender parts. This makes a T-spin double donation.

You can upgrade it further by placing L over it and making a Z overhang. This becomes a DT-STSD. Make the T-spin double.

You get a partial leftover STSD.
5. 8-1 Stacking Situations
When you are 8-1 stacking (the garbage hole being in columns 2 or 9), you have many chances to make DT-STSDs:

Step 1 shows a regular T-spin double with the garbage hole in column 2.

You get this uneven stack.
This can easily be topped with 2 Js to make a DT-STSD.
Doing this, however, requires piece awareness and planning.

Memorizing these common permutations improves your chances of successfully creating DT-STSDs mid-game.
In a Nutshell
DT-STSDs, having 3 T-spins inside, are sometimes dangerous because of the heavy upstack. Like any complex setup with more than 2 T-spins, one has to forsake them for simpler setups if one is heavily pressured.

Ends with a clean T-spin double.
Provides a massive burst of 3 T-spin doubles.
Has many mid-game situations, especially in 9-0 and 8-1 stacking.
Cons
Dangerous and bulky; you could get spiked while creating it mid-game.
